We will be transparent in how we operate as a collective

We value multiple worldviews and perspectives

We agree that trust and relationship is key to the success of C5

We use a shared leadership model to govern our action

We will be responsive to the environment and emergent issues

We will learn from the community we serve and act on community wisdom.

We are risk takers and will not be silenced

THEORY OF CHANGE

Increasingly communities are facing pressures that have a negative impact on their vibrancy.

The change we want is to:

Work with people in a holistic way, across all the social determinants of health.

Respond to each individual’s needs, hopes and culture with a system that builds relationships and puts each individual at the center of decision making and service delivery.
